If you'd like to arrange a telephone consultation with one of our GPs, fill in this form. You can easily attach photos or supporting documents to this form if needed. 

This form should NOT be used for medical emergencies, eg: chest pain, symptoms of stroke, acute injury, sudden headache or loss of vision, etc. Call us on 014022300 if you require urgent care or see this page for out of hours information. Surgery hours are Monday to Friday 9am - 12:30pm and 2pm - 5pm. We are closed on all Bank Holidays. See home page for opening hours over any holiday period. 

Once we have received and processed your form, you will be sent a text message with an appointment date and time, during our normal surgery hours. If this form is submitted outside of our normal opening hours, it will not be reviewed until the Practice reopens. 

If you would like to speak to a specific doctor, please bear in mind that wait times may be longer. 

If you do not have a medical card, the standard fee for a private patient is €65 and is payable on the day of your consultation. We will send you a secure link to pay online. See this page for all fees.
