Chronic Disease Management Programme

If you have one or more long term medical conditions and hold a medical card, you may benefit from regular review. We are inviting eligible patients to register for the Chronic Disease Management (CDM) Programme, a new initiative which has been rolled out across the country in Primary Care. This programme has been designed so that doctors can work together with their patients to manage current health conditions and address any concerns.

The programme takes place twice a year, and consists of an initial 45 minute appointment with one of our nurses, including fasting blood tests, followed by a 15 minute appointment with a GP two weeks later. There is no cost to patients to participate in this programme.
